If you submit a request for an appointment outside of these hours, they will respond on the next working day.

Your details

You need to provide a referral from your practitioner to book an appointment.

Don’t have a doctors referral?

If you don’t have a referral from a doctor, it’s highly likely we won’t be able to see you. Please chat to your doctor to discuss referral options.

Have a referral but can’t upload it?

That’s okay. We will need to contact you to discuss your appointment, so make sure you keep an eye on your email or phone. You’ll need to bring your referral with you on the day.

Cards

To confirm your billing, we need to know if you have a Pension Card or a Health Care Card. Seniors cards are not accepted.
